Community centers and an older adult’s social network are the key to identifying which older adults might be at risk for falls or becoming homebound. As of 2022, HOP-UP-PT is now providing community center staff, employees, and volunteers the opportunity to become a Certified HOP-UP-PT Ambassador. There is no cost to becoming a certified ambassador.
Certified HOP-UP-PT Ambassadors are essential in identifying individuals in their communities who may be at-risk for a health decline and may in need of the HOP-UP-PT fall prevention program. The training consists of watching a short online video to learn about the HOP-UP-PT program
As a Certified Ambassador, you will:
- Serve as a leader in the HOP-UP-PT community. Our Ambassadors serve as the key to community center referrals by recognizing a potential health decline before it occurs.
- Help older adults stay safe, active, and healthy in their golden years by connecting with HOP-UP-PT physical therapists
- Stay up-to-date on the latest HOP-UP-PT news and information
